Visit requests go to the Optimize U clinic team so they can contact you about scheduling. Seeker signups are used to send Field Notes. Questions sent to Ask Derek may be answered on the show or in Field Notes, by first name only and only if you said that's okay.

We don't sell your information. Please don't send medical details through any form on this site. Share those at your visit.
